Position Summary:Responsible for the dispensing and monitoring of all medications provided through the Hospital Pharmacy (inpatient and outpatient). Applies sound professional principles and judgment the dispensing and monitoring of pharmaceuticals to assure patient safety and compliance with all Federal, State and Hospital laws, rules, regulations and procedures. Supervises support personnel assigned to their areas of responsibility. Fills patient medication orders (including compounding whenever necessary), prepares small and large volume parenterals, chemotherapeutic agents, and sterile and non-sterile preparations, floor stock requisitions, chargeable and non-chargeable items stocked in patient care areas. Performs, supervises, coordinates and/or approves all pharmacy departmental
services as assigned including:
Filling of alcohol, narcotic and other controlled substances orders;
Processes and replaces emergency and stat trays, and other par level replacement items;
Performs patient care area inspections, documentation and follow-up procedures;
Refreshes stock, checking for expired/contaminated/mislabeled/deteriorated items. Removes and processes returns in accordance with department policy. Credits items awaiting final disposal;
Processes requisitions to drug manufacturers or wholesale distributors to replenish stock supplies. Renews and/or distributes stock with appropriate documentation. Alerts supervisory personnel to low stock situations.
Packages drugs and pharmaceutical products including sterile formulations.
Keeps abreast of contemporary pharmacy practice including providing drug information concerning rational drug utilization, renal dosing, adverse drug reactions, monitoring, medication error reporting, recognizing major drug interactions, IV compatibility, IV to PO conversions, precautions and investigational drug requirements.
Adapts to and integrates advances in technology as they relate to integrated patient-oriented services; pricing and charging of pharmacy items to patients, employees, physicians, and other customer groups.
Effectively counsels outpatients on the use of Rx/OTC medications;
Effectively communicates with peers, other healthcare professionals and patients (including ability to write medication orders under physician supervision);
Takes the initiative to communicate potential problems to supervisory personnel and other care team members.
Education:B.S. Pharmacy
Licensure:Current RI Pharmacist Licensure required.
